*ADDRESS:

30 Gainsborough Drive, Halesworth, IP19 8TL.

I am in the second turning on the right in Gainsborough Drive (first turning is Constable Close). I am in a Cul-de-sac. There is a Poppy outside the front door, and I have a red garage door (which you should see as you turn into the cul-de-sac).

 

 

*PARKING:

There is a car park opposite the turning into my Cul-de-Sac. It is a bit hidden as you have to drive between the only lamppost and big tree, and over the foot path between the houses. There are bins there on one side, and a children's play area too. Please use the car park, and I ask to not park in front of the houses if possible as this should be used as a turning square, and may block driveways.

 

 

*PAYMENT:

I do not currently own a card machine at the moment, but accept cash or bank transfer (BAC must be paid PRIOR to treatment). Due to no ease of access to a bank, I can no longer accept cheques.

 

 

*CONSULTATION:

When you arrive, we will go through a consultation form prior to the treatment. Please allow extra time for this, as this means we will be finished later than the given treatment time. The consultation is not included in the treatment time and so extra time will be required.

 

Please remember to bring reading glasses if necessary, as this consultation form will need to be signed and dated.

 

 

*ADDITIONAL CONSENT/MEDICAL NOTE

Additional medical consent notes may be required, and are to ensure your safety.

 

If you have an underlining health condition, then a Doctor or medical note may be required prior to your treatment at Poppy’s Cabin. Likewise, if you are currently receiving Oncology treatment or check-ups, then a Doctor or medical note WILL be required prior to treatment.

 

Poppy’s Cabin will happily send a letter via email or post to you (postage at no extra charge), for you to obtain a date and signature from your Doctor / medical team prior to your treatment at Poppy’s Cabin. You will also be required to sign and date this note. This note will explain my qualifications, contact of my Oncology training body (if necessary), and information of the treatment.

 

It is at Poppy’s Cabin’s discretion if a signed medical note is, or is not required. If a signed medical note is not required, you may still be asked to sign a personal consent form in addition to the general consultation form.

 

Please contact Poppy’s Cabin if you are unsure if you will require a medical note. As stated previously, this is to ensure your safety and obtaining your Doctor/medical team’s consent. Please be assured that Poppy holds qualifications in all treatments that are offered at Poppy’s Cabin, and holds up to date insurance so that these can be performed and carried out safely.
